I think the heat spreader is soldered to the chip. Therefore, you must lap both surfaces if they are not smooth and flat.

Also check the temp at stock vs overclocked settings. If there is a huge discrepancy, then the HS is not making good contact. If the change in temp is small, then the temp sensor is wrong.
Yea,
checked using orthos at 4200mhz,
run for 30min, temp went from 68c to 95 and stayed there.
from stock vcore to 1.4 for 4.2ghz the temp difference is around 6c.

so seems the ihs/core seems alright and the temp sensor is off.
