WTF with the reference cooler ?

Normally, people learns from the mistake they made but it seems AMD don't.
How is it possible on earth that AMD repeat this kind of failure ?
Do they really like to be on top of the charts regarding noise @launch ?
Why present your flagship product with such a flaw ?
