How to set up GTL Ref Values for 45nm & 65nm

On the Asus Maximus Formula II for the CPU GTL it has 4 (0-3) ref's to set. The first one uses the formula with 0.635 and then it alternates with 0.667 for the rest of the time. So it looks like this.

0.667
0.635
0.667


Thats just the formulas it states. So I'm confused as to what to set. It's +40mV for the first one and 3rd one but what about the other two?

For Vtt around 1.2v, I use
