I have a Zalman 160XT that looks pretty
could you just attach your smart card reader to a rear usb port and leave it under the case, then pull it out when you need it ?
apart from that, zalman are nice.
I cool mine with a single 90mm fan running slow, they case has a big vent above the CPU to allow heat to rise straight up
