At a loss..and need some help.

Well I figured it out! It's grounding somewhere in the case it seems. I took the whole setup and and set it up on the mobo box and it works every time.

I guess I need to buy some of those washers for the motherboard screws. Aside from that is there anything else I can do for potential grounding to the case?
