Okay something is officially strange. I was always running 185.xx drivers because with 182.xx I've got black screens on boot. Now I gave the 182.50 a chance, this time I've left the dummy off. Installed the driver, rebooted. Switched SLI off and left PhysX enabled, rebooted. Booted with the dummy still off, and not only do I get to see my desktop with this config, no, I was even able to extend the desktop to the HDMI port (screen 3) without the dummy! Vista64 with no dummy, no switching around, just boot, extend and run the GPU clients....
Did I miss something here?
Edit: Rebooted now again, but this behaviour was not repeatable. I'm still running without dummy, but that's just because the dummy didn't work anymore. Plugged it in, tried to extend but it didn't stick. So I left the dummy off and tried disabling enabling PhysX and now I'm up and running again on both cores. Without dummy/monitor. Could it be that nV drivers are inconsistent crap?
