I know the band is Ishtar, they're a Swedish black metal outfit, but I've never seen that cover art.

Correct! It's Ishtar and the album is Krieg. Smashing Black Metal. Love'em. It's a long story, but - in most forums my nick is Abyss, and there's that song in the album. Supreme, pure black metal. And I like the art - has some dark, depressing beauty.
