UTT RAM survey: dead or alive?

I just wanted to conduct a database and poll to see how many people have had the UTT sticks went belly up, It would be helpful if we can get the following information:

mobo:
Color slots: (orange or yellow?)
bios:
5v or 3.3v rail:
vdimm used:
speed:
24/7 machine: (yes or no)
1 or both sticks:
ram's age:
ram model: (OCZ VX, Mushkin Redline, etc.)
