Exclamation UTT RAM survey: dead or alive?

I just wanted to conduct a database and poll to see how many people have had the UTT sticks went belly up, It would be helpful if we can get the following information:

    PSU:
    mobo:
    Color slots: (orange or yellow?)
    bios:
    5v or 3.3v rail:
    vdimm used:
    speed:
    24/7 machine: (yes or no)
    1 or both sticks:
    ram's age:
ram model: (OCZ VX, Mushkin Redline, etc.)

IF your UTT still alive...please fill in the following as well:

    PSU:
    mobo:
    Color slots: (orange or yellow?)
    bios:
    5v or 3.3v rail:
    vdimm used:
    speed:
    24/7 machine: (yes or no)
    ram's age:
ram model: (OCZ VX, Mushkin Redline, etc.)

Edit: this survey is for UTT ram only, TCCD and any other ram need not apply.

    PSU: OCZ Powerstream 520
    mobo: DFI NF$ SLI-DR
Color slots: (orange or yellow?) Orange
    bios: 310
    5v or 3.3v rail: Both
    vdimm used: 3.2v
    speed: 250 24/7
24/7 machine: (yes or no) Not any more. It was the last 2x512 set-up I used before going to trying 2gig set-ups. My old BH-5 and TCCD actually outperformed this TMSP AA4T (CH) slighty on benches. I have four sticks of 1A4T I'm trying to sort through right now.
    ram's age: 3 months.
ram model: (OCZ VX, Mushkin Redline, etc.) TMSP AA4T (CH)

    I voted yes.

    PSU: PCP&C 510 SLI
    mobo: DFI Ultra D
    Color slots: orange
    bios: 510-2
    5v or 3.3v rail: 5v
    vdimm used: 3.5v
    speed: 250 2-2-2-5
    24/7 machine: yes
    1 or both sticks: 1
    ram's age: 2 days
    ram model: Mushkin Redline XP4000

2nd pair is doing great so far at 250 with only 3.1v on 3.3v rail.

Edit: jeesus.....I know the ratio might not be as accurate as it seems at the moment but 30% failure rate is pretty high!!

    Yes, 2 sets actually

    PSU: OCZ 520w Powerstream

    bios:310
    5v or 3.3v rail: 5v

    speed:260mhz
    24/7 machine: yes
    1 or both sticks:1 stick
    ram's age:under 3 months
ram model: OCZ VX PC3200 Gold

    PSU:Antec Neopowe 480w
    mobo:MSI K8N Neo2 Plat
    Color slots:
    bios:
    5v or 3.3v rail:OCZ DDR Booster

    1 or both sticks:1 stick
    ram's age:under 2 months
ram model: OCZ VX PC3200 Gold

I think the problem on the Neo2 was just a fluke, but the DFI board was my own fault for using the 5v rail. It's kinda funny, I had read the thread on DFI boards killing UTT just the day before my ram died.
