Phase change mount - where to buy replacement?

I bought a phase change unit last year. It worked great until this year when the plastic mounting hardware broke. Not sure if it was over tightening or just long term extreme cold temperature exposure.

Anyway, I need to replace it (probably along with the foam as it's getting pretty compressed these days).

Anyone know where I can pick up one of these that will fit my cooler?
