Sounds like to me, it was a drunk fork lift driver. As far as the horn, if it had a mercury contact that set it off when tilted more than 45° angle, it should work.
I know they have a small plastic device that staples to boxes, that looks like a small beeker (chemistry thingie) that when the box is tilted too far its releases a dye that changes the color of the liquid inside.
For the crushing aspect of it, unless you use those strong plastic crates, you would amlost have to weld together a small cage for it. I have seen this done way back when I worked in shipping and recieving. Customers where tired of getting there stuff all banged up, so they devise the cage method for extra security. And no, even these didn't prevent all mishaps.
