nv4.dll is a driver for Nvidia cards

could be overheating of your card or corruption of your vram
maybe try to use the latest drivers and see if it solves sth

Most likely your memory on your system. Try lowering the speed on it.

Well, I was playing cs:s for awhile, and then all of a sudden. That "Choppy noise" and BSOD.
I thought..what the hell right? just go back to playing, but this time, JUST incase set my fan speed to 75%
happens about 5mins after game play.
Then after that BSOD, I try again, but this time sources freezes twice, and im like errr...
So I google it quickly, and someone said they downclocked there 8800gtx alittle, for the record my card isn't overclocked
so I lowered it to 650 core, and it played fine..
I believe I have the latest drivers :-/

It's not unheard of for vid drivers to become corrupted. Try a complete removal and reinstall, it might help.
